Completion of this form on-line should take approximately 5-10 minutes, and is a requirement for use of a FACIT questionnaire (confidentiality will be maintained). However, it is not intended as a method of requesting information or copies of the scales. You may view and download copies of all English version FACIT scales at view questionnaires and translation availability, or request other information under the registration and request section of this website.
