Click on "view language availability" to view the status of available language translations for each corresponding scale. Click on "download sample copy" to access a PDF sample copy of the corresponding questionnaire.
Click on "register for English download" to access a user ready version of the corresponding scale in English. Follow the instructions to register or login in order to gain access to this component of the website. While there is no charge for access to the English version of these measures you must register and log-in before you can download a user-ready copy of a FACIT scale.
