- Register yourself with this site:
If you would like to register yourself with this site, go to register to login. You will be asked to provide your name and contact information, and to create a personal password in order to login as a registered user. Logging on to the site provides you with download capability of English questionnaires (no payment), and the ability to purchase rights and access restricted areas of this site. - Register your use of a FACIT questionnaire:
If you would like to register your use of a questionnaire in a project or trial, go to collaborator project information form. This is required for use of any FACIT questionnaire. Please keep in mind that completion of this form does not serve as a request for information. - Request information or purchase materials:
If you would like to request information or purchase FACIT Measurement System user material (e.g., full website access, a binder manual, or a scoring diskette), please go to one of the following links under "requests": Purchase materials or website access, consulting estimate, letter of permission, translated scales.
If your request is for access to protected areas of the website, you will be able to view and download information immediately after you have submitted the registration and request information and agreed to any acceptable fees. Requests that involve a response from our staff or the processing of hard copy materials will typically be handled within 3-5 business days.
An invoice for any purchased products (including access to password protected sections of the website) will be sent to you through your e-mail account. The invoice will include all of the appropriate remittance information and will be the only invoice you will receive. We very much appreciate your prompt payment by check.
