MICRA Multidimensional Impact of Cancer Risk Assessment For patients who have undergone genetic testing for risk of cancer LICENSE THIS MEASURE Overview Language Availability Licensing Selected References Related Measures Overview Overview The Multidimensional Impact of Cancer Risk Assessment (MICRA) is a tool to measure the specific impact of result disclosure after genetic testing. The authors compared its performance with that of questionnaires measuring general and cancer-specific distress. Participants (158 females) responded 1 month after they received genetic test results. They were divided into 4 standard clinical test result groups: BRCA1/2 positive, BRCA1/2 negative, panel negative, and true negative. Factor analysis supported the formation of 3 subscales: Distress, Uncertainty, and Positive Experiences. All 3 MICRA subscales differentiated participants who were BRCA1/2 positive from the other 3 groups. MICRA thus helps identify subgroups of vulnerable genetic testing participants. Original development and validation involved 854 patients with cancer and 15 oncology specialists. An initial pool of 370 overlapping items for breast, lung, and colorectal cancer was generated by open-ended interviews with patients experienced with the symptoms of cancer and oncology professionals. Using preselected criteria, items were reduced to a 38-item general version. Factor and scaling analyses of these 38 items on 545 patients with mixed cancer diagnoses resulted in the 27-item FACT-General (FACT-G). Coefficients of reliability and validity were uniformly high. The scale's ability to discriminate patients on the basis of stage of disease, performance status rating (PSR), and hospitalization status supports its sensitivity. It has also demonstrated sensitivity to change over time. Its development followed FDA guidelines for the patient-reported outcome measures, which emphasizes improvement in tumor-related symptoms or delays in symptom progression as evidence of clinical benefit for patients in oncology drug trials. Methods consistent with the FDA guidance were utilized along with prior experience developing 11 National Comprehensive Cancer Network (NCCN) symptom indexes to identify the highest priority symptoms and concerns of patients with GINET, PNET, and LNET, and of patients with CS, to inform the development of a set of NET symptom indexes appropriate for use in both clinical trial and clinic settings.
